Affinity Skilled Living and Rehabilitation Center is a 280-bed skilled nursing and rehabilitation facility on Locust Avenue in Oakdale. It cares for two groups: patients who need short-term, sub-acute rehab after a hospital stay, and residents who need long-term nursing care around the clock. Rehab programs include physical, occupational and speech therapy, with a focus on orthopedic, neurological, cardiac and pulmonary recovery. A dedicated respiratory unit supports ventilator-dependent residents and tracheostomy care, including weaning programs. Other services include IV therapy, wound care, dental, podiatry and audiology visits on site, lab and X-ray, meal planning by a registered dietitian, therapeutic recreation, and social work with discharge planning. Rooms are private or semi-private, each with its own bathroom. The facility accepts Medicare, Medicaid and many commercial and managed care plans, and it reviews Medicaid-pending referrals case by case. Families are welcome to visit at any time.
