About
Compassionate Healthcare Management & Advocacy (HCM Advocacy) is a nurse-led, fee-for-service company that helps older adults manage their health and find their way through the healthcare system. It was co-founded by Michele Barraco, RN, BHA, CSA, its Chief Clinical Officer, who has 35 years of experience in bedside nursing, home health care and physician practices. Clients are paired with a registered nurse who works with them, or with a family member or chosen representative, by phone seven days a week. Service packages cover chronic disease management (health assessments, medical record and medication reviews, education and coordination with doctors), transitions home after a hospital stay, and elder care planning, including guidance on hospice, palliative care, assisted living, long-term care and home care. Services are sold as monthly packages or individually, including the PRI assessment New York requires for nursing home admission and as-needed nurse consultations.
