Livewell Care Management

LiveWell Care Management, formerly Alighieri Care Management, provides geriatric care management for older adults and their families from an office on West 22nd Street in Manhattan, with a second New York office in Larchmont serving Westchester. Its Aging Life Care professionals begin with a full assessment of medical, emotional and social needs and then build a care plan. Nurse advocates can go with clients to medical appointments, oversee medications and keep track of health changes, while care managers coordinate appointments and keep providers in communication. The team also helps families select home caregivers, guides moves to assisted living, memory care or skilled nursing, and offers life care planning that ties together care, finances, insurance, entitlements and veterans benefits. Other services include round-the-clock crisis support, help with bills and benefits, and counseling for seniors and relatives. Rebecca Lippel is the Director of Care Management.
