About
Next Chapter Home Transitions guides seniors and their relatives in Suffolk County and across Long Island work through housing and care transitions. Its main service is an in-home safety evaluation for people who want to keep living at home. During a visit of about two hours, the home is reviewed for fall hazards, mobility barriers and everyday accessibility problems, and the family receives a prioritized, room-by-room report with photos, suggested changes and a next-step checklist. Families often request it after a fall, before a hospital discharge, or when stairs and bathrooms become a worry. The service does not replace inspectors, therapists or contractors, and it connects families with elder law, financial, care and housing resources. Founder Bruce Schneider is a Senior Home Safety Specialist and Senior Real Estate Specialist (SRES) with 30 years of home renovation experience. A free 15-minute call is the first step.
