About

Smithtown Center for Rehabilitation and Nursing Care is a 164-bed skilled nursing facility on North Country Road in Smithtown, in a Victorian-style building on more than ten landscaped acres. It offers both short-term rehabilitation and long-term care. Patients recovering from surgery, stroke, cardiac problems or illness receive individualized physical, occupational and speech therapy, along with respiratory therapy, IV therapy, wound care and planning for the return home. Long-term residents have nursing care around the clock, help with daily activities, restorative nursing, tracheotomy care, and management of diabetes, pain and nutrition. Dentists, ophthalmologists, podiatrists and wound specialists see residents on site, and lab work, x-rays and ultrasound are available in the building. Amenities include a rehab gym, recreation and music programs, a salon and a courtyard. Admissions liaison Peg Callaghan and the admissions team help families and hospitals arrange transfers, and tours can be scheduled in person or virtually.
