About
YHP Cares is a service coordination agency, approved by the state Department of Health, for adults who have active Medicaid and need a nursing home level of care but want to keep living at home. It serves people 18 and older in the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) waiver programs, including seniors and brain injury survivors. Under President Samantha Lawson, the team works with each client and family on a care plan, sets goals for building or keeping independent living skills, and coordinates supports that reconnect the person with community life. When difficult behaviors make daily life harder, the agency uses a positive behavior intervention approach to ease routines at home. YHP Cares also helps families think through Medicaid planning for long-term care, including how to qualify while protecting assets, and offers a free Medicaid planning checklist.
